Appointments more than 24 hours from the scheduled appointment time can be rescheduled or canceled for a full refund.

Appointments less than 24 hours from the scheduled appointment time cannot be rescheduled and are non-refundable if canceled.

Certified Lab Services "Mobile" sends a mobile phlebotomist to you to collect your diagnostic testing. Cost for any mobile collection will be $80.00 USD. We come to you to the location of your request.

Certified Lab Services "Site Location". If you come to our location its only $35.00 USD for any Lab collection order.

Therapeutic Lab Draws are $150.00 USD for mobile services.

The cost of laboratory testing is separate and charged by the laboratory (e.g. Labcorp or Quest). To make things easier for you, we offer the ability to upload your insurance information. If you choose to upload it, we will provide it to the laboratory so they can process your bill using insurance. To check the exact price of your laboratory testing bill, we recommend calling the laboratory ahead of time.

Certified Lab Services charges an out-of-pocket convenience fee starting from $35 and up. In certain cases, you can submit your receipt to your insurance provider for reimbursement. Once your specimens are delivered to the laboratory you select or request, the testing itself is generally covered by insurance. We recommend checking first to avoid any surprises. To find out if your insurance is accepted by your preferred lab. If you chose not to submit your insurance information we are not liable for any covrage or missed or incorrect insurance information at is submitted.

Lab results will typically be ready in 3-5 business days though certain tests may take longer. Once your lab results are ready, your doctor will typically reach out 2-3 days after to review the results together. In addition, you can check if your lab results are ready with the following options:

1. Check the patient portal for your laboratory

Grail

2. Connect your Apple Health app with your laboratory account

3. Contact your doctor's office 5 business days after your visit

For your privacy, Certified Lab Services does not receive any information on your lab result status - only you, the provider who ordered your labs, and the laboratory you selected will receive your lab results.

No lab order no problem. We have four easy options for providing your lab order to Certified Lab Services when booking a visit. 

1.You can provide us your doctor’s contact information and we can collect your lab order for you.

2. You can ask your doctor for a copy of your lab order and submit it to us or walk-in with your lab order.

3. You can have your provider fax over the lab order to : 480-809-0057

4. You can have your provider email your lab order to: Contact@certifiedlabservices.com
